The recommendation stems from a preliminary investigation into the San Jose National High School shooting, which left three students dead and several others injured.
Prosecutors said the findings point to negligence that contributed to the tragedy, underscoring accountability within law enforcement ranks as the case moves toward formal charges.
According to investigators, Pacencia’s alleged failure to properly secure firearms played a role in enabling access to the weapons used in the shooting.
